Use this form to request an order form for a video of an event that you or a member of your family participated in.

Why do we ask for so much information just to send you an order form? (Read why, below form.)

Please note that we require all the information requested below in order to be able to accept your request.

Please use yyyy/mm/dd format
For a school event, enter the school name. For a Dance, Equestrian, or other competition, enter the event name (E.g., "Dance Devotion" or "Lord Dewdney Challenge Games"). For all other events, enter the name of the studio/club/company/society that organized the event.
The Participant is the Grad, Dancer, Rider, Actor, Singer etc. who appears in the video that you wish to buy. This individual must be a member of your household.

Required only if event was a competition
Enter the name of the studio/club/company/society that they represented in the competition.

  Why do we ask for so much information just to send you an order form?

People's appearance on video is protected under certain circumstances by privacy legislation. As well, most of the participants in the events that we video are minors. So we do our best to ensure that a person asking for an order form is entitled to order the DVD that they say they want to. We do this by assessing how much information the requestor knows about the event they're expressing an interest in.
